Try wiggling the connector to the monitor when the problem occurs. I don't think it will help but just give it a try.
I think its probably your graphics card. If it was top end a couple of years ago then the processor on the graphics card will probably have a cooling fan on it, but they are crap and seize up.
Once they seize its only a matter of time before the chip overheats and blows and you get the sort of thing you've got. The good news is graphics cards of that type are cheap now.
